How Is the Care Plan Created and Reviewed?
A care plan is a detailed document that outlines exactly what support a person needs, how it should be delivered, and when. It is the foundation of safe, effective home care and ensures that every carer who visits understands the person's needs, preferences, and routines.
The care plan is developed following an initial assessment, which is usually carried out in the person's home by a care manager or senior carer. During the assessment, the care manager will discuss the person's daily routine, health conditions, mobility, cognitive function, dietary needs, medication, and personal preferences. Family members are encouraged to be involved in this process.
The resulting care plan covers all aspects of daily living that require support โ personal care, medication, meals, mobility, social activities, and any specialist needs. It also records important information such as emergency contacts, GP details, allergies, and communication preferences.
Care plans are living documents โ they should evolve as the person's needs change. Regular reviews, typically every four to six weeks or sooner if circumstances change, ensure the plan remains accurate and relevant. Reviews may be triggered by a change in health, a hospital admission, or feedback from the carer, the person, or their family.
